I guess most use blender for a model done from scratch.
There is also PMX editor.

Blender is great. There are plugins for exporting to .pmx. There's also Xismo which is MMD-oriented, don't use Metasequoia, it might have been good years ago but right now it's a poor choice regarding its lack of features.
